Sample Acceptance Conditions

  1. Please fill out the application form completely. Requests for addition/reduction/correction to the requested tests and specified information are made written before starting the laboratory test process. Change requests not made written will not be considered; in this case, tests, pricing and billing are done in line with the original information.
  2. There must be a stamp and authorized signature of the applicant company on the test request form. Responsibility for faulty stamp and signature belongs to the applicant company. In the absence of a stamp, an authorized signature is required.
  3. The test process is not started for the requests containing incomplete information and insufficient samples. The day the missing information and/or sample is completed is considered the first business day for the relevant request in our laboratory.
  4. Samples that have not been processed due to lack of information, sample shortage or waiting for payment are kept by us for a maximum of 1 week. We are not responsible for the samples that could not be processed or recovered by the applicant within this period.
  5. When sample return is requested, samples must be taken from our laboratory by the applicant within one week. It is sent by courier upon request, with a counter payment.
  6. The second copy of the applicant form is for the applicant to keep for reference. The report number of the application is notified to the applicant together with the second copy of the request form after the test request is accepted and numbered. The follow-up of the test requests in our laboratory by the applicant is only done with this report number.
  7. The test request must be cancelled by the applicant within 24 hours at most. Cancellation requests later than this period will not be considered.
  8. Test reports are only and always forwarded to the applicant, the representative companies written on the form, and the companies specifically specified by the applicant in the test request form. No information is given to persons or organizations other than this, except with the written consent of the applicant. Kazlıçeşme Laboratory is obliged to keep the information of its customers confidential. Kazlıçeşme Laboratory informs the customer in advance about the information it wants to make available to the public. Except where the information is made public by the customer or there is an agreement between Kazlıçeşme Laboratory and the customer, all other information is considered private information. However, in cases where Kazlıçeşme Laboratory is authorized to disclose confidential information, legally required or contractually authorized, the customer is informed of the information to be disclosed, unless prohibited by law. Information about the client obtained from sources other than the client will remain confidential between the client and the laboratory. The source of this information is kept confidential in Kazlıçeşme Laboratory and is not shared with the customer unless approved by the source.
  9. After the test, the remaining samples are kept for 15 days.
  10. Unless there is an agreement to the contrary, reports are delivered by printing only once.
  11. Reports are prepared in Turkish or English at no extra charge. Unless specified at the time of application, requests for reports in another language are subject to a separate fee stated in our price list.
  12. In case the invoice addressee is different from the applicant company, it should be notified written that they accept all the conditions stated in the test request form.
  13. If the customer delays / fails to fulfil the payments, Kazlıçeşme has the right not to prepare a new test report before all payments are made to the them.
  14. Kazlıçeşme is responsible to its customer for the testing services it has outsourced. The customer provides the test service to be subcontracted is carried out with the written test request form or e-mail, and with the approval of the customer.
  15. Kazlıçeşme testing services do not assume any responsibility in the event of non-performance or delay due to reasons such as natural disaster, war, terrorism, government or state restriction, any limitation by law, strike, lockout, malfunction in machinery, fire, accident.
  16. In the event of any complaint, Kazlıçeşme's responsibility cannot be more than 5 times the contract price, which is the subject of the complaint, under any circumstances.
  17. Istanbul Courts and enforcement offices are authorized in disputes between the parties regarding this service contract.
  18. The recommended minimum sample quantities for any test request are given in the Table below. In case of hesitation, the applicant can apply to our laboratory and get guidance for the required sample.
  19. Tests marked with “**” are those that will be subcontracted if the customer approves.
  20. Tests marked with “*” are analyses accredited by TÜRKAK.
  21. For samples delivered until 14:00 and taken on site by Kazlıçeşme staff until 12:00, that day is considered the first working day.
